What is an ABA Therapy SOAP Notes Template?

SOAP Notes are commonly used around the world in a wide variety of healthcare settings, including Applied Behavior Analysis (ABA) Therapy. SOAP is an acronym that stands for the four headings used in SOAP Notes: Subjective, Objective, Assessment, and Plan. How to Use This SOAP Note Template for ABA Therapy

This template is very simple to use. The structure of the SOAP Notes for ABA template is just four headings, and for our ABA Therapists who travel to all different locations outside of an ABA clinic, we have added a space for the venue of the session as well as the time and duration of the session. These are all important details to include in your session notes and may become important in the future for legal or insurance purposes.

Once you have filled in the header information such as the patient’s name, session venue, and date, begin filling in the SOAP headings making up the body of the ABA Therapy SOAP Notes template.

Subjective

Information under the subjective heading may include your initial impression of your client’s presentation or anything reported to you by the client or support person relevant to their present condition.

Objective

The objective heading is for your reporting of the facts of the session. What did you do or say? What was your client’s response? How did you intervene? The ABC acronym can be useful here, standing for Antecedent- what happened before a behavior, Behavior- what did your client do, and Consequence- the response following the behavior.

Assessment

The assessment section is where you write your conclusions based on the information gathered in the first two sections.

Plan

And finally, the plan section is your next steps for your client as their ABA therapist- most likely including when and where you will see them next, and any plans for their future sessions.

Who Can Use this ABA Therapy Template?

This ABA therapy template is designed for use by anyone who routinely practices ABA therapy. This might be a:

  • Board-Certified Behavior Analyst (BCBA) or Board-Certified Assistant Behavior Analyst (BCaBA)
  • Registered Behavior Technician (RBT)
  • Trainee or student for either of the above.

Trainees may find using a template a good way to learn the SOAP Notes structure, whereas certified ABA practitioners or behavior technicians can incorporate this template into their regular note-taking routine.

Where do I put information reported to me by my client’s parent/caregiver at the time of the session?

Information you are told that you did not observe or measure yourself, goes in the subjective section of the SOAP Notes. This might be your client directly reporting their feelings to you, or information about the client given to you by a support person.

I’ve been taught the ABC method for session notes, is this an alternative to SOAP?

Not necessarily. ABCs can still be incorporated into your SOAP Notes and can be really useful in structuring your notes in the objective section of the SOAP Notes.
